Enhanced Family Care (EFC) provides adults with developmental disabilities and acquired brain injuries the opportunity to live in a safe, welcoming family home while receiving individualized support. Unlike a traditional residential setting, individuals become part of a family environment where they can build meaningful relationships, develop independence, and participate in their communities. At Siddharth Services, Inc., we believe the best support begins with truly knowing the person. Each placement, support plan, and daily routine is designed around the individual’s strengths, preferences, cultural background, interests, and personal goals. Every home is certified through the State of New Hampshire and supported by our experienced team to ensure high-quality, person-centered care.

What Enhanced Family Care Includes:

Personalized Home Matching

We take the time to match each individual with a Home Care Provider whose home, lifestyle, and interests best support their needs, preferences, and personal goals.

A Safe & Supportive Home

Individuals become valued members of a household while maintaining their privacy, independence, and personal routines. Depending on the home and individual’s needs, an individual may share the home with another person receiving services. Roommate arrangements are considered carefully to promote compatibility, safety, respect, and a positive home environment.

  • A private, furnished bedroom personalized to the individual’s needs and preferences
  • Comfortable shared living spaces that encourage connection and participation
  • Internet, phone, and cable/streaming services
  • Safe heating, cooling, and well-maintained living spaces
  • Nutritious meals and snacks based on dietary needs and preferences
  • A state-certified home regularly monitored for safety, quality, and compliance

Person-Centered Support

Support is based on each individual’s:

  • Interests and hobbies
  • Communication style
  • Cultural preferences
  • Daily routines
  • Medical and behavioral needs
  • Personal goals

Support plans evolve as each person’s needs, interests, and goals change.

Health & Wellness

Our Home Care Providers support each individual’s health and well-being by helping them maintain healthcare routines, access services, and communicate with their support team.

Support may include:

  • Medication administration and monitoring
  • Scheduling, attending, and following up on medical, dental, and therapy appointments
  • Coordination with healthcare providers and other professionals
  • Ongoing health monitoring and follow-up care
  • Nutritious meals and snacks that reflect individual needs and preferences

Community Inclusion & Exploring New Places

We believe everyone should have opportunities to be active in their community, explore new places, and participate in activities they enjoy.

Support may include:

  • Shopping and dining out
  • Local events and attractions
  • Fitness and recreation
  • Religious or cultural activities
  • Visiting friends and family
  • Volunteering and employment opportunities

Transportation

Transportation is provided for approved activities and services, including:

  • Visits with family and friends
  • Siddharth Services, Inc. classes and social events
  • Day trips and approved travel or vacations

We work with the individual, Home Care Provider, and state to identify appropriate transportation options, including specialized vehicles when needed.

Our team works together to keep support consistent, responsive, and focused on the individual’s needs and goals.

Home Care Providers receive ongoing training and participate in regular home visits, quality assurance, and collaboration with the Siddharth Services support team to maintain state certification and provide high-quality care.
