A Provider Agency delivers direct supports and services. Siddharth Services is a Provider Agency offering Residential Services (Enhanced Family Care), Day Services, Respite, Behavioral Consultation, Nursing, and Clinical Services.

Yes. We welcome referrals from individuals, families, guardians, Area Agencies, schools, healthcare providers, and other community partners.

Enhanced Family Care is our residential service model where individuals live with carefully matched Home Care Providers who offer individualized supports in a family setting that promotes independence, stability, and community inclusion.

Our Day Services provide individualized community-based activities, life skills, wellness opportunities, employment exploration, and meaningful experiences based on each person’s goals.

Respite Services provide temporary care and support while giving caregivers time away. Services are individualized based on each person’s support plan and needs.

Each participant receives individualized supports through their service plan to encourage independence, community participation, skill development, and personal growth.

Yes. We provide individualized, person-centered supports for individuals with behavioral, clinical, and medical needs.

Behavioral Consultation Services use evidence-based, person-centered strategies to improve communication, independence, daily routines, and positive behaviors through individualized planning, staff education, and collaboration.

Our nursing team provides medication administration training and certification, oversight, quality assurance, documentation review, and health education to support safe medication practices and regulatory compliance.

Yes. Our nursing team provides 24-hour on-call support for urgent medical concerns outside normal business hours.

Yes. Our nurses provide education and guidance on medical conditions, medications, and ongoing health needs to support quality care.

Yes. When appropriate, we collaborate with clinicians, therapists, physicians, case managers, and other members of the individual’s support team to promote consistent care.

Yes. Transportation varies by program and the individual’s Individual Service Agreement (ISA).

Safety is our highest priority. We use individualized support plans, comprehensive training, required background screenings, ongoing supervision, and continuous quality oversight to provide safe, person-centered services.

All Home Care Providers complete state-required training, certification, background screenings, and ongoing education before providing services.

We encourage you to first speak with your Program Manager. If additional support is needed, your Program Manager Supervisor is available to assist.

For urgent situations outside normal business hours, our on-call Program Manager and on-call Nurse are available to provide support.

We prioritize regular communication with individuals, families, and guardians. Please contact your Program Manager with any questions or updates regarding services.

Yes. We encourage families and guardians to participate in planning and ongoing communication, based on the individual’s preferences and applicable consent.

Yes. We regularly review support needs and can adjust services as needs, goals, or circumstances change.
